By agreeing to these Terms and Conditions, the customer acknowledges and accepts the outlined
responsibilities and scope of work.
Scope: General conditions for the small job (jobsite protection, daily clean-up, coordination). Remove the ceramic tile and backer board in the kitchen (120 sq ft) and
level the sag zone under the refrigerator (floor leveler or plywood — method and extent confirmed
once the tile is up). Supply and install cork-backed waterproof LVP in the kitchen (120 sq ft) at
the $2.00/sq ft material budget; a higher-priced selection at Floor & Decor is billed as the
difference. Install one long transition at the kitchen doorway (~10 ft) and quarter round / shoe
molding where baseboards are covered, primed and painted to match. Refrigerator and dishwasher are
moved out and reset; the floor register is reinstalled as-is.
Not included: Carpet in the other rooms (planned separately — "one floor at a
time"), cabinet / countertop / sink work (they stay in place), structural or crawlspace work
(crawlspace already inspected and verified), new tile, plumbing or electrical changes, moving
furniture or personal items beyond the refrigerator and dishwasher, and work outside the
kitchen.
Contractor supplies: LVP flooring at the $2.00/sq ft budget allowance, floor
leveler / plywood, transition molding, quarter round / shoe molding and paint to match, fasteners
and adhesives.
Notes:
**Jobsite will be left clean once completed.
**Refrigerator and dishwasher are moved and reset by the contractor; the contractor is not
responsible for pre-existing appliance conditions.
Warranty:
**12 months workmanship.

· Deposit to Schedule: $731.25 — pay online with the Accept & Pay Deposit button above (credit card)
· Final Payment: $731.25 — due upon completion
